assistance limits & paperwork

Assistance limits are as follows (all rates are per individual per year):

• $100: required info is name, phone number and address to be filled in Donation Distribution Document, see below

• $500: required info is copy of national ID + evidence of low income (examples) + signature on money Cash Receipt Document, see below

• Up to $12,000: national ID + evidence of low income + purchase receipts. Receipts to be submitted via Receipts Form, see below

• For nonprofits, paperwork required is official nonprofit registration & work portfolio (once a time). Then donation receipt w/ each donation

• All applicants individuals or organizations need to clear OFAC list

For new applications use New Applicant or مقدم طلب جديد to submit first time assistance request. Otherwise, use Current Applicant
